Giving us feedback
Patient feedback provides a valuable insight into what services are like for patients, carers and families. Your experience and feedback is important to help us inform improvement and service design for the future.
There are a number of ways you can provide us with your feedback:
Our online feedback form
You can contact us by completing our online feedback form – use the blue button on the left of this page.
Friends and Family Test
Friends and Family Test
The Friends and Family Test (FFT) is a national survey and a way for all patients to give feedback about their care and treatment.
You can take part in the survey at the time of your discharge home from hospital, or after an appointment or home visit. If you have not already done so, you can also complete the survey here as soon as possible after receiving care or treatment.
To access a FFT survey, please scan the correct barcode from the list below or follow the corresponding link.
CQC National Patient Survey Programme
CQC National Patient Survey Programme
The Department of Health initiated a national patient survey programme to be carried out by all NHS Trusts. These surveys cover all areas of care. If you’ve had a recent stay in hospital, you might receive a copy of the survey through the post.
The purpose of these surveys is to listen to what patients feel about the services provided by hospitals and take steps to make improvements wherever problems are identified.
The Care Quality Commission (CQC) provides a report showing the results for each Trust throughout the country. The most recent CQC survey reports for Airedale NHS Foundation Trust can be viewed here.
Patient stories
Here at Airedale NHS Foundation Trust, we want to know what matters to you, in your own words.
By listening to your stories, we can learn what we are doing well and what needs to get better which will help us to improve our services and patient experience.
Any patient, staff member, family member or carer can tell their story. Telling your story will not have an impact on you or your care. You are welcome to remain anonymous.
We will decide with you where you are happy to share your story. You can change your mind at any time about where your story is shared by letting us know. Your story may be shared with the Board, across the Trust or with teams and individuals. We usually tell the team involved about your story so they can learn from it.
If you have a story about our services that you would like to share please contact us.
